Survey and Evaluation Forms

Gathering reflections on your program from involved students, teachers and department volunteers is an important tool in evaluating the successes of the program and improving it over time. Street Law has designed four simple reflection forms which should be modified to fit each program, and we’ve posted samples from other corporations below. One page forms are ideal, especially for students, who might have limited time and concentration to complete their reflections.
